Assume that you are a case manager for a human service organization in your area.

 

Determine the needs and services the client in Appendix D requires.

 

Create a viable care plan for the client profiled in Appendix D.

 

Consider that in order for your plan to be successful it must meet all of the needs of your client, regardless of whether the services exist in your area or not. If the services are not available in your area, you will also need to provide a plan for how those services will be acquired.

 

Write your care plan for this client in 700 to 1,050 words.
